Customers served (external and internal): all participants, any member of the LifeROOTS Team, board members, all vendors, suppliers and any other members of the public.  

Role Summary:

Principal Accountabilities: (essential job functions):

  1. Monitor and observe LifeROOTS’ Participants in employment/work settings.  Assists with job    start-up by providing one on one job coaching, as needed.
  1. Teaches participant job skills, requirement utilizing job task analysis, job specific supports including assistive technology; and educates employers/co-workers regarding participants with disabilities including one and one and group presentations.
  1. Analyzes job requirements and break into manageable components that can be taught to the participant. Also, assesses potential job sites for possible adaptation/accommodations and brainstorm appropriate assistive technology or adaptive equipment for that site.
  1. Completes Vocational Assessments and is responsible for timely completion of the participant’s job development activities in accordance with their formal or informal job development plan; participates in Vocational Profiles as appropriate and coordinates job development activities which relate specifically to the vision of each participant.
  1. Contacts community employers about possible employment opportunities for participants served; provides information on and/or negotiates with employers regarding job restructuring/carve outs as appropriate.
  1. Maintains accurate and detailed documentation of all job development activities in accordance with LifeROOTS policy, DD Waiver standards and State General Fund standards; completes all job development and Service Coordination documentation including, but not limited to, Progress Notes, Contact Notes, internal/external billing forms and other forms as applicable.
  1. Assists in development of a transition plan from initial coaching to long-term coaching and follow along by Employment Specialist I/II.
  1. Build natural support network between the participant and co-workers.
  2. Communicates effectively with lead, supervisor, LifeROOTS Service Coordinators, employer representatives, and other relevant parties; and participates in departmental projects including all staff meetings.
  1. Acts as advocate for service recipients and liaison with all parties involved with service recipients. Communicates regularly with LifeROOTS Service Coordinators and other members of the IDT regarding services.
  1. Presents program in a positive manner to the public.
  1. Flexible in responding to varied job assignments - nights, weekends and holiday work may be required.
  1. Provide personal assistance, as needed, and able to according to limitations/doctor’s restrictions.
  1. Complies with DD Waiver, Mi Via, State General Funds, DVR and Centennial Service Standards.
  1. Ensures that consumer transportation to job development activities has been arranged and/or transports service recipients, as needed.
  1. If transporting a Participant, must be in done in LifeROOTS Company vehicle and must have written authorization about Participant, destination and purpose of the trip signed off by Director of Adult

Community Services.

  1. Identify/solve problems before they become a crisis for the participant, employer and co- workers.
  1. Maintains a positive solution-oriented attitude about all services provided to participants served.
  1. Secures funding where appropriate and prepares reports as needed.
  1. Coordinates, schedules, and conducts Participant Service Plans (ISP’s), annual and 6 month reviews; assessments etc., as needed.
  1. Conducts intake and orientation on new service recipients.
  1. Assists service recipients in securing services from other programs and agencies.
  1. Processes eligibility for services recipients.
  1. Responds to telephone inquiries regarding records.
  1. Participates in training opportunities relevant to job duties.
  1. Responsible for initial training about participants needs service by LifeROOTS.

Custodial Crew Leader-CABQ

Role Summary:

Custodial Crew Leader must provide leadership that directs and manages Team members and work processes during the shift.  Custodial Crew Leaders supervisory responsibilities include directing all Team members working in the building during the shift.

 

Principal Accountabilities: (essential job functions):

1.       Ensures that basic cleaning and periodic cleaning are done according to the contract.

2.       Enforces and follows the guidelines of the quality control plan.

3.       Ensures that all time sheets are properly completed and turned in to the supervisor in a timely fashion.

Role Summary: Ensures the effective day to day operations of: Career Discovery, Literacy, Menaul and Rio Rancho Day Programs. Programs Manager will hire and train supervisor staff, provide supervision and support to all staff in areas related to Community Inclusion services/activities. 1)     Oversees Day Habilitation Programs and provides informed reports to Director of Adult Community Services.

Role Summary:  Responsible for the over-all coordination and day-to-day administration of the Children’s and Therapy Services Program.  Responsible for assuring the provision of quality services in keeping with the most current, best practices in Early Childhood Services including  therapy services for children outside of early intervention, early intervention and in compliance with funding and monitoring standards.
