Role Summary: Responsible for the management and supervision of maintenance and repair workers at Pine Tree Contracts. This position will provide guidance and direction to personnel and is responsible for ensuring that the facilities are fully and well maintained.

Employment Support Coordinator
Description:  

Customers served (external and internal): all participants, any member of the LifeROOTS Team, board members, all vendors, suppliers and any other members of the public.  

Role Summary:  

mso-layout-grid-align:auto">Functions as support to Employment Team, providing the service of advocating, identifying, developing, coordinating and accessing supports and services on behalf of individuals or assisting individuals and their families to access employment supports and services.

mso-layout-grid-align:auto">Though the main duties of this position will be Employment Support Coordinator, this position must cross train with the Service Coordinator, Job Coaches and Employment Specialist I Job Descriptions. This position will eventually transition into full-time Service Coordinator.

 

Principal Accountabilities: (essential job functions):

  1. Monitor and observe LifeROOTS’ Participants in employment/work settings.  Assists with job    start-up by providing one on one job coaching, as needed.
  1. Teaches participant job skills, requirement utilizing job task analysis, job specific supports including assistive technology; and educates employers/co-workers regarding participants with disabilities including one and one and group presentations.
  1. Analyzes job requirements and break into manageable components that can be taught to the participant. Also, assesses potential job sites for possible adaptation/accommodations and brainstorm appropriate assistive technology or adaptive equipment for that site.
  1. Completes Vocational Assessments and is responsible for timely completion of the participant’s job development activities in accordance with their formal or informal job development plan; participates in Vocational Profiles as appropriate and coordinates job development activities which relate specifically to the vision of each participant.
  1. Contacts community employers about possible employment opportunities for participants served; provides information on and/or negotiates with employers regarding job restructuring/carve outs as appropriate.

Maintains accurate and detailed documentation of all job development activities in accordance with LifeROOTS policy, DD Waiver standards and State General Fund standards; completes all job development and Service Coordination documentation including, but not limited to, Progress Notes, Contact Notes, internal/external billing forms and other forms as applicable.

Literacy Coordinator
Description:

Role Summary: Lead, Coordinate and monitor participant and programs in the literacy program in accordance with LifeROOTS mission, vision, and values.  Responsible for developing and monitoring a Literacy and Career Readiness curriculum to provide a full day of meaningful, active programming based on participant preferences,

 

 

Principal Accountabilities: (essential job functions):

1.      Provides individualized classroom and community experiences that promote literacy skills- development and maintenance, including career readiness activities for participants.

 

2.      Develops, assists, implements, modifies and evaluates the Literacy curriculum which includes career readiness activities, outcomes and posts desires within participant sight. Part of the development will include supporting participants to track their own outcomes.-

 

3.      Maintains a current curriculum for the Literacy Program, while using a diverse selection of assistive technology, educational tools and resources.

4.      Create a curriculum on advocacy that must be addressed 2 hours a week that will include creating a power point presentation, guest speakers, etc.

 

5.      Conducts and documents monthly participant meetings, develops a community inclusion calendar of activities for the program that is Literacy related, community-based and appealing to the interest of participants served.

 

6.      Coordinates and participates in community inclusion outings on a daily basis, as required by State Standards. 

 

7.      Responsible for funds related to program and participant accounts, including but not limited to spending, documenting, training staff and balancing for financial audits.

 

8.      Communicates with participants, IDT Teams, Supervisors, Service Coordinators, Case Managers and Nurses regarding participant progress and growth.

 

9.      Schedules and conducts tours at educational and employment sites based on choices made by participants.

Role Summary: Under close supervision, perform routine custodial activities as assigned.

  • Duties include, but are not limited to, vacuuming floors, sweeping and wet mopping floors, cleaning commodes, sinks, moving furniture, and cleaning urinals; cleaning walk-off mats, removing trash, low dusting, cleaning glass, drinking fountains, spot cleaning, re-supplying the restrooms.
