Principal Accountabilities: (essential job
1. Secures funding where
appropriate and prepares reports as needed.
2. Coordinates, schedules,
and conducts Individual Service Plans (ISP’s), annual and 6 month reviews.
3. Conducts intake and
orientation on new service recipients.
4. Assists in transition
planning and crisis intervention.

19. Responsible for participation
in the company’s quality improvement program and ensures that all
department-applicable quality improvement requirements are fulfilled.
20. Responsible for participation in the company’s financial management and
ensures that all department-applicable fiscal responsibilities are fulfilled
