Community Discovery Mentor
Role Summary:  The CDM provides transportation and support to identified participants, individually or in small groups as they explore the community as they relate to their goals.  Goals can include but are not limited to discovering potential employment opportunities, building and engaging in relationships in the community and/or learning and accessing services provided in the community.


Principal Accountabilities: (essential job functions):

1.       Follow documented plan regarding days, times and places services is being provided.

2.       CDM is responsible for picking up participant as identified in plan. This may mean at the participant’s home or other identified no facility related space.

3.       CDM will complete the daily trip safety vehicle check list for their personal car or company car.

4.       All vehicle safety check lists will be turned into the Employment Team Supervisor weekly.

5.       Completes required paperwork accurately and timely according to departmental procedures.

tab-stops:58.5pt">6.       Communicates effectively with supervisor, LifeROOTS Service Coordinators, employer representatives, and other relevant parties.

7.       Participates in the development of the participant’s Individual Service Plan (ISP).

8.       Presents program in a positive manner to the public.

9.       Provide personal assistance, as needed, and able to according to limitations/doctor’s restrictions.

10.   Build natural support network between the individual and the public.

11.   Identify/solve problems before they become a crisis for the participant(s), individuals in the community and possibly other relevant relationships to the participant.

12.   CDMs will cross train with other CDMs, Job Coaches and possibly CICs to ensure that seamless coverage is being provided to the individual we serve. 

13.   Attend all program, divisional, organizational and identified participant meetings unless prior arrangements have been made with the department head.

14.   Submit for approval mileage sheets as instructed to supervisor the last Friday of every pay period.

15.   Complies with DD Waiver, Mi Via and State General Funds Standards.

16.   Maintains conduct that is professional and ethical at all times.

17.   Complies with all applicable LifeROOTS policies and procedures.

18.   Relies on instructions and pre-established guidelines to perform the functions of the job.  Primary job functions do not typically require exercising independent judgment.

19.   Responsible for participation in the company’s quality improvement program and ensures that department applicable quality improvement requirements are fulfilled.

20.   Responsible for participation in the company’s financial management and ensures that all             department-applicable fiscal responsibilities are fulfilled.

21.   Responsible for participation in the company’s Health and Safety program and ensure that all             department-applicable safety requirements are fulfilled.

22.   Performs other related duties as assigned by the Employment Team Supervisor.

Essential Skills and Experience:

1.     HS Diploma or GED.

2.     Successful completion of all required DOH and LifeROOTS organizational trainings.

4.     Must complete individualized trainings as required by the participant’s plan.

Beneficial Skills and Experience:

1.   College education or diploma.

  1. CDM should have strong interpersonal and communication skills.
  2. Time Management: Proven ability to manage one’s own time and the time of others well. 

4.    Proven practice of monitoring/assessing performance of oneself, other individuals, or organization to make improvements or take corrective action. 

5.    Experience working in high performance teams and performing as a proven strong team player.

6.    Actively looking for ways to help people. 

7.    Commitment to company values.

8.     One year of experience working with people with disabilities.


Conditions of Employment:

1.       Fingerprints and background check.

2.       A valid/current driver’s license with good driving record.  Current auto insurance.

3.       Routinely operates a company vehicle or a personal vehicle while on company business.

4.       Personal vehicle must meet safety inspection requirements on every trip.

5.       Must be at least 18 years of age.


Physical Challenges:

1.       Regular and consistent use of keyboard and mouse.

2.       Physically able to perform essential functions of position without hazard to themselves or others. 

3.       The work is non-sedentary, requiring walking, assisting in lifting and transferring clients, bending, and lifting carrying 50 pounds.

4.       Exposure to changing weather conditions.

5.    Must have the capacity to toilet and provide personal care with people with disabilities.

6.       Assists with wheelchair transfer of non-ambulatory participants.
