Role Summary: The CDM provides transportation and support to
identified participants, individually or in small groups as they explore the
community as they relate to their goals.
Goals can include but are not limited to discovering potential
employment opportunities, building and engaging in relationships in the
community and/or learning and accessing services provided in the community.
Principal Accountabilities: (essential job
1. Follow documented plan
regarding days, times and places services is being provided.
2. CDM is responsible for
picking up participant as identified in plan. This may mean at the
participant’s home or other identified no facility related space.
3. CDM will complete the
daily trip safety vehicle check list for their personal car or company car.
4. All vehicle safety check
lists will be turned into the Employment Team Supervisor weekly.
5. Completes required
paperwork accurately and timely
according to departmental procedures.

development of the participant’s Individual Service Plan (ISP).

Essential Skills and Experience:
1. HS Diploma or GED.
