Day Program Supervisor - Rio Rancho

Role Summary: Responsible for developing and monitoring plans to provide a full day of active programming based on individual preferences. 

 

Principal Accountabilities: (essential job functions):

1.       Trains and supervises all assigned program staff in specific areas of Community Integration Coaches job descriptions including, but not limited to, meaningful community outings, vehicle monitoring, public relations, safety requirements, quality control, etc.  Develops program and services based on the individualized needs of the service recipients.

2.       Responsible for all aspects of supervision of the Community Integration Coaches including, but not limited to, hiring, discipline, training, ongoing supervising and scheduling in accordance with LifeROOTS policies and procedures.

3.       Responsible for orientation and initial training of new Community Integration Coaches.

4.       Determines ongoing training needs of Community Integration Coaches.

5.       Provides accurate data and information to interdisciplinary teams by attending ISP’s and other meetings as well as through other forms of communication such as telephone, e-mail, and any other communicative method.

6.       Ensures all paperwork on service recipients is completed accurately and on a timely basis.

7.       Ensures that individual plans are implemented and/or modified as necessary.

8.       Keeps accurate contact information for work performed.

9.       Meets daily with all assigned staff to assist in the implementation of program planning and the sharing of pertinent information regarding the consumer’s daily activities.

10.   Meets at least monthly with all program participants and maintains accurate documentation on all assigned meetings.

11.   Transports persons served in agency vehicle when necessary.

12.   Ensures the proper usage of all program equipment and supplies.

13.   Ensures that documentation for billing purposes is accurate and submitted on time.

14.   Conduct performance evaluations on a timely manner.

15.   Maintains a clean, sanitary environment that is appealing and representative of a professional atmosphere.

16.   Delegates housekeeping tasks to appropriate staff, overseeing the implementation of those tasks on a daily basis.

17.   Assists with the development and implementation of departmental goals and objectives.

18.   Oversees and controls inventory and reorders supplies as needed for operation of the program.

19.   Ensures that all monies are used ethically and monitored on a daily basis.

20.   Maintains positive and professional relationships with all stakeholders and community at large.

21.   Represents the agency in a professional manner.

22.   Establishes effective and professional relationships at all organizational levels.

24.   Complies with DD Waiver Standards and State Fund Standards.

26.   Exercises personnel authority with respect to employment, training, salary changes, performance appraisals and termination of assigned staff members.

27.   Responsible for participation in the company’s quality improvement program and ensures that all department - applicable quality improvement requirements are fulfilled.

28.   Responsible for participation in the company’s financial management and ensures that all department - applicable fiscal responsibilities are fulfilled.

29.   Responsible for participation in the company’s Health and Safety program and ensure that all department - applicable safety requirements are fulfilled.

30.   Performs other related duties as assigned by the Director of Community Services.

Essential Skills and Experience:

1.       Must have a Bachelors degree in social service or related field; Three years of experience working with adults with developmental disabilities, experience may be considered in lieu of education.

2.       Experience working with adults with behavioral challenges desired; must complete required training.

3.       Computer literacy a must.

Conditions of Employment:

1.       Fingerprints and background check.

2.       Must have a valid and current driver's license with good driving record and must have current auto insurance as required by applicable state law.

3.       Routinely operates a company vehicle while on company business.

4.       Must be at least 18 years of age.

 

Physical Challenges:

1.       Regular and consistent use of keyboard and mouse.

2.       Physically able to perform the essential functions of the position without hazard to themselves or others. 

3.       The work is non-sedentary, requiring walking, assisting in lifting and transferring clients, bending, lifting, and carrying 50 pounds.

4.       Must have the capacity to toilet and provide personal care attendant service to people with disabilities.
