Role Summary: Under general direction
manages and directs nursing care activities and coordinates care planning with
other disciplines.
Principal Accountabilities: (essential job
functions):
1. Manages/directs nursing
care activities and coordinates care planning with other disciplines.
2. Provides nursing care to
clients.
3. Assesses health needs
and obtains necessary interventions.
4. Administers medications
and treatments per physician's orders.
5. Provides and maintains
verbal communication and written documentation to assure health needs of
clients are met.
6. Performs general nursing
care to clients including administering prescribed medications and treatments
in accordance with nursing standards.
7. Assesses individual's
health status and reaction to drugs and treatments and communicates with
physician and/or department management.
8. Orders medications and
supplies as needed.
9. Maintains and reviews
individual's records and pertinent medical information.
10. Must be able to document
observations and assessments onto designated medical records.
11. Assists in formulating
and implementing health and hygiene goals in the Individual Service Plan (ISP)
and provides health-related teaching as indicated.
12. Prepares and presents
training pertaining to Direct Service personnel (i.e., assisting with
Medication Delivery, Health Care plans, Medical Response Emergency Plans, etc.).
13. Adhere to State
licensing requirements, DOH standards and DD Waiver requirements.
14. Compiles with all
applicable LifeROOTS policies and procedures.
15. Relies on instructions
and pre-established guidelines to perform the functions of the job. Primary job functions require exercising
independent judgment.
16. Responsible for
participation in the company’s quality improvement program and ensures that all
department-applicable quality improvement requirements are fulfilled.
17. Responsible for
participation in the company’s financial management and ensures that all
department-applicable fiscal responsibilities are fulfilled.
18. Responsible for participation
in the company’s Health and Safety program and ensure that all department-applicable
safety requirements are fulfilled.
19. Performs other related
duties as assigned by the Director of Adult Community Services.
Essential Skills and Experience:
1. Associates degree and
LPN or Bachelor of Science degree and RN.
2. Must be licensed to
practice nursing in the State of New Mexico as a Licensed Practical Nurse (LPN)
or Registered Nurse (RN).
3. At least one year of
professional nursing experience required
4. Computer experience in
word processing and spread sheet programs; able to compose reports and letters.
5. Participation in recent
continuing education programs related to Community Services, per licensing
requirements.
6. Fundamental
understanding of LifeROOTS, Inc. policies.
