Physical Therapist

Customers served (external and internal): all clients, any member of the LifeROOTS Team, board members, all vendors, suppliers and any other members of the public.  

 

Role Summary:  Physical Therapist provide services that help restore function, improve mobility, relieve pain, and  prevent or limit permanent physical disabilities of patients suffering from injuries or disease. The Physical Therapist restore, maintain, and promote overall fitness and health. The Physical Therapist’s patients include accident victims and individuals with disabling conditions such as low-back pain, arthritis, heart disease, fractures, head injuries, and cerebral palsy.

Principal Accountabilities: (essential job functions):

1.       Evaluates and performs physical therapy services.

2.       Works as a trans-disciplinary team member to evaluate and implement strategies to address outcomes on IFSP’s and ISP’s.

3.       Maintains required paperwork including daily notes, progress reports and evaluation reports.

4.       Prepares specialized equipment, apparatus or materials for clients as needed.

5.       Maintains expected level of efficiency.

6.       Performs therapy at multiple sites, i.e. at the center, at home, at daycare, etc.

7.       Acts as resource for families providing information, support and community resources.

8.       Provides consumer with written instructions for home follow through.

9.       Complies with standards of ethics established for Physical Therapists.

10.   Complies with all applicable LifeROOTS policies and procedures.

11.   Exercises personnel authority with respect to employment, training, salary changes, performance appraisals and termination of assigned staff members.

12.   Responsible for participation in the company’s quality improvement program and ensures that all department - applicable quality improvement requirements are fulfilled.

13.   Responsible for participation in the company’s financial management and ensures that all department - applicable fiscal responsibilities are fulfilled.

14.   Responsible for participation in the company’s Health and Safety program and ensure that all department - applicable safety requirements are fulfilled.

15.   Performs other related duties as assigned by the Children’s and Therapy Services Manager.

Requirements
Essential Skills and Experience:

1.       Graduation from accredited physical therapy school with a minimum BS degree; State of New Mexico licensure required.

2.       Experience in pediatric and physical dysfunction settings for minimum of one clinical affiliation.

3.       Provides services for children in home, and at LifeROOTS.

4.       Participation in recent continuing education programs related to Children’s Therapy Services, per licensing Requirements.

5.       Fundamental understanding of LifeROOTS, Inc. policies.

Beneficial Skills and Experience:

1.       Physical Therapist should have strong interpersonal and communication skills.

2.       Time Management: Proven ability to manage one’s own time and the time of others well.

3.       Monitoring: Proven practice of monitoring/assessing performance of oneself, other individuals, or organization to make improvements or take corrective action.
